Creating a form
You build a form once for your whole team. It then appears by itself at the place you choose, for every coach in the team.
Where a form appears
A form belongs to one place in the app. Today those are the steps of a test:
- Lactate test: the General information step or the Protocol step
- Metabolic Performance Profile: the General information step or the Protocol step
Each place has one active form. When you activate another form on the same place, Coachbox asks whether the previous one may be archived. An archived form keeps its answers, and you can activate it again later.
Creating or editing a form
- Go to Account > Forms
- Click New form, or start from an example
- Add fields with Add field
- Under Where this form appears, choose the step it belongs on
- Set the status to Active and save
Fields come in categories:
| Category | What |
|---|---|
| Structure | Sections to organise your form |
| General | Text, numbers, choices, dates, scales |
| Measurements | Measurements with a fixed unit, such as waist, FVC or resting heart rate |
| Metrics | Fields that also go into the athlete's metrics |
| Calculated | Values Coachbox works out, such as BMI or age |
What each field does, how calculated fields work and what metrics fields write through is in Field types.
How to put a form on a test step and what happens to the answers is in Forms in your tests.
Good to know
- Forms belong to the team, not to the coach who made them. Every coach in the team uses the same forms.
- Forms also work for test persons without an account.
